Health & Wellbeing

At The Swans Community Partnership, we are committed to improving the health and wellbeing of individuals across Milton Keynes through inclusive, accessible, and community-led support.

We aim to build long-term, collaborative partnerships with local NHS services, working together to reduce health inequalities and deliver better experiences and outcomes for those most in need.

Social Prescribing

We work closely with GPs, local medical centres, and the Integrated Care Board to support social prescribing pathways. By connecting individuals to our wide range of community-based activities and services, we help people improve their physical and mental health, build social connections, and take greater control of their wellbeing.

Schools & Organisations

Our tailored wellbeing programmes support young people who may benefit from a more focused approach to physical, social, and emotional development. These interventions help build resilience, confidence, and positive habits—laying the foundation for lifelong health and learning.

Community Activities

We offer a variety of inclusive activities for adults across Milton Keynes, designed to:

  • Increase physical activity and reduce obesity

  • Improve mental and physical health

  • Boost resilience and emotional wellbeing

  • Reduce social isolation and loneliness

  • Our goal is to create a healthier, more connected community - where everyone has the opportunity to thrive.

Falls Prevention & Active Ageing

We are committed to helping older adults remain active, independent, and connected within their communities. Through low-impact physical activity, strength and balance programmes, walking groups, and social activities, we support individuals to maintain mobility, improve confidence, reduce the risk of falls, and enhance overall wellbeing.

By combining physical activity with social engagement, our programmes help tackle loneliness and isolation while enabling older people to stay healthier, more active, and independent for longer.

We use sport, physical activity, and community engagement to improve physical health, mental wellbeing, and quality of life. Our programmes help people become more active, reduce social isolation, build confidence, and develop healthier lifestyles, creating positive outcomes for individuals, families, and communities.

We recognise that wellbeing is about more than physical activity alone. Through supportive, inclusive, and community-led opportunities, we help people improve resilience, strengthen social connections, and access activities that support both their physical and emotional wellbeing.

 

Personal Development

Our programmes create opportunities for people to develop confidence, leadership skills, resilience, communication, teamwork, and independence. Whether through volunteering, mentoring, training, or participation in community activities, we support individuals to realise their potential, build positive relationships, and progress into education, employment, and wider community leadership opportunities.
